BONTOC, Mountain Province - Processing of passport applications would be done in this capital town on April 5 as part of events to commemorate the province’s foundation day.
Executive assistant Roger Sacyaten said personnel of the Department of Foreign Affairs in Manila, through its mobile passporting system, will be in Bontoc that day for final processing of passport applications and to personally interview qualified applicants.
Sacyaten told applicants to submit complete supporting documents at the provincial governor’s office on or before March 24 for submission to the DFA for encoding and initial processing.
The DFA’s mobile passporting system will process passports for new applicants, renewal of passports and replacement of lost passports.
For new applicants, the following documents are needed: Birth certificate issued by the National Statistics Office and certified true copy of birth certificate issued by the municipal local civil registrar duly authenticated by NSO; NBI clearance; 3 pieces 4.5cm x 3.5cm ID picture (w/ royal blue background); marriage certificate (original copy) for married women applicants, also authenticated by the NSO and a certified true copy of the marriage contract from the LCR, and any proof of identity indicating full name, date and place of birth and citizenship.
For renewal of passport, photo copy of the old passport, pages 1, 2, 3 and the amendment inside and back cover are needed.
The old copy is presented to DFA; birth certificate authenticated by the NSO and a certified true copy of the same issued by the LCR; 3 pieces 4.5cm x 3.5cm ID pictures (royal blue background); marriage contract with security paper authenticated by the NSO and a certified true copy of the same issued by the LCR.
For lost passports, the same requirements as in the new applicants plus a notarized affidavit of loss and additional supporting documents like IDs are required to establish the applicant's identity. – PIA-Mountain Province
